Personal care assistance is the foundation of in-home support, helping individuals with the everyday tasks that maintain hygiene, health, and dignity. At Prime Community Care, our trained caregivers provide respectful, hands-on support that allows clients to remain safely at home.
Through Colorado’s Health First Colorado (Medicaid) and the Community First Choice (CFC) benefit, eligible individuals can receive personal care assistance at no out-of-pocket cost. Our team works closely with case managers and care coordinators to ensure each client receives the right level of support.
Our caregivers are trained to provide respectful, professional assistance across all personal care needs.
Full or partial bathing, hair washing, oral hygiene, skin care, and personal grooming to maintain cleanliness and dignity.
Assistance selecting and putting on clothing, grooming hair, nail care, and maintaining a neat, comfortable appearance.
Safe assistance with walking, position changes, transferring from bed to chair, and use of mobility aids and equipment.
Discreet, respectful assistance with toileting, incontinence care, and use of related equipment and supplies.
Timely reminders and support to help clients take their medications as prescribed, supporting health and routine.
Caring presence and conversation during personal care visits to support emotional wellbeing and reduce isolation.
Our services are available to eligible individuals enrolled in Health First Colorado (Medicaid).
